For the new curriculum, Michelle Brosseau has written up a data-gathering framework that is based on Building Thinking Classrooms and Grading for Equity.

Also, Amanda Lutty has one that she altered.
And a colleague of Paige Nicole showed her the x and circle method. When having conversations or observing a student. She uses a printed list and put the expectation at the top, students get a \ if they have a basic understanding, an X if it is sufficient and an X with a circle around it (⦻) if they have a more advanced understanding. It has helped me a lot with my grade 9's. If they have absolutely no idea she puts a period . She doesn't do the full class at once but tries to get different students each time she does it.
